About the Medical Questionnaire

A medical questionnaire will be provided at reception when you visit our clinic.
However, to make your check-in smoother, you may also print it from the link below, complete it in advance, and bring it with you.

Our obstetrics and gynecology services are provided by certified specialists affiliated with the Departments of Obstetrics and Gynecology at Showa Medical University and St. Marianna University School of Medicine.

Pediatrics

Our pediatric services are provided by board-certified specialists in neonatology and pediatrics from the Department of Pediatrics at St. Marianna University School of Medicine.

Consultation Schedule: Once a month
Friday: 09:00 – 12:00, 14:00 – 17:00

We provide care for conditions associated with pregnancy and childbirth, as well as consultations for non-pregnant patients.
No appointment is required for internal medicine consultations.
Due to the nature of our obstetrics clinic, we are unable to see patients with symptoms of contagious illnesses, such as colds or gastroenteritis.
